A) State Road Occupational Medical Facility is located just south of Lake Erie on State Road, near the intersection of State Road and East 6th Street. Map
Q) Is it difficult to access needed services?
A) Appointments may be scheduled during normal business hours, 7:30am-4:30pm Monday-Friday. Many patients appreciate having the ability to make same day appointments. Same day appointments fill up quickly, so we recommend that you call early in the day. If you cannot keep a scheduled appointment, please call 24 hours in advance. Arriving late for you appointment will delay others and you may be asked to reschedule. While we try to accommodate walk-ins whenever possible, we ask that you please make an appointment even if it is only to call ahead to make sure we have room in the schedule. If you arrive without calling, you are taking the chance we might be booked for the next few hours or even the whole day. If you walk in without an appointment, we need to see the people who have appointments before we treat you, so you might have to wait longer than you or we would like.
In Case of Emergency: Go to the nearest hospital emergency department if you have chest pains, difficulty breathing, serious injury, or bleeding.
Q) Are your certified?
A) State Road Occupational Medical Facility is approved by the Bureau of Workers Compensation. We also meet all state and local requirements.

A) The medical services your company will receive will be both innovative and of the highest quality, yet we will be fair and competitively priced. Office visits are $60.00, please call for any other pricing.
Q) How do I implement the Drug Free Work Place program, and where do I start?
A) Professional and knowledgeable staff are available to assist you during initial implementation all the way through the program’s completion. These services are available as a local resource for the businesses that State Road Occupational Medical Facility serves.
Q) How long do drug screen results take to get back?
A) Typically, a negative drug screen result will be reported within 24-48 hours.
Q) I am an employer, how can I retrieve my drug screen results?
